Connect5 Athy Survey
Connect5Athy is a collaborative project between the Royal College of Surgeons Ireland and Sláintecare Healthy Communities Athy.
It aims to build a smartphone app to support healthy choices around sleep, diet, stress management, daily activity, substance control, relationships, and other lifestyle issues.
If you are aged 18+ and live, work and/or play in Athy, we are inviting your input. Please complete this anonymous and secure survey by Monday 31st October 2022:
You can also complete a paper copy of the survey at Athy Library or use a tablet there to aubmit a digital response.
